This referral form is designed to support young people who are disengaged or at risk of disengaging from mainstream education. It enables schools and agencies to provide detailed background information and supporting documentation to help assess a student’s eligibility for enrolment at a Special Assistance School. Please ensure all relevant documents, such as recent school reports and learning plans, are included to avoid delays in processing.
